Care Management
The part most agencies skip.
Care management is the work of actually running a care plan: coordination, oversight, communication, adjustment. It's the difference between a caregiver showing up and a family feeling like care is handled. It's included in every CareQuilt plan.

Most agencies give you caregiver hours. What they don't give you is someone making sure those hours are actually working. So the job of managing care — noticing patterns, coordinating with the doctor, updating the family, adjusting the plan when things change — falls on whoever has the most time. Usually the adult daughter. And it wears her down.
We flipped this. Care management isn't an add-on — it's the backbone of every plan. Your care manager knows your family. Every plan gets clinical review. You get regular updates. When something changes, the plan changes with it. That's the service.
A dedicated care manager who knows your family — one person for everything.
Regular updates on what happened, what's changed, and what we're watching.
Clinical review of caregiver observations, with patterns flagged over time.
Coordination with your family doctor, home-care nurse, specialist, or memory clinic when something needs attention.
Care plan reviews with your family to adjust goals and cadence as needs evolve.
If something comes up between visits, your care manager is already in the loop and knows what to do.
Every family we work with. Care management isn't an upgrade — it comes standard. When you work with us, you're not just getting a caregiver. You're getting the team behind them.
Some families already have a caregiver they love and just want the management layer on top. We do that too — and it comes with the added benefit of proper insurance, liability coverage, and tax documentation. Mention it on the first call.
